COMMUNITY COLLABORATION FOR CHILDREN

Community Collaboration for Children (CCC) has three components: A Regional Network of providers, In-Home based Services and Family Team Meetings. CCC serves families in Kenton, Campbell, Boone, Pendleton, Grant, Gallatin, Carroll, Owen, Scott, Bourbon, Harrison, and Nicholas counties.

The Regional Network meetings are comprised of local community partners from family and child-serving agencies as well as parents, local citizens, and schools. The Regional Network meets monthly to make funding and programming decisions about activities that focus on child abuse prevention. The Regional Network has funds for faith based, fatherhood, and April Child Abuse Prevention Month activities and events.

In-Home Based Services are intensive, short-term services that occur in the home and focus on a comprehensive treatment plan to address the family’s unique needs. An In-Home Based Service Provider may work with a family for up to 16 weeks. Services will focus on empowering the family unit by teaching problem-solving skills and coordinating available community resources.

Family Team Meetings are meetings that bring families, agencies, and community partners together to resolve issues that exist within the family. The purpose of a Family Team Meeting is to make a family-driven plan with assigned tasks and timelines that addresses the family’s needs with the assistance of a trained facilitator who will lead the meeting. In a Family Team Meeting, everyone is given respect for their ideas and input toward helping the family achieve their goals.

To receive services, the children must be in the custody of the parent and the family must be low-risk. Referrals will be looked at on an individual basis to determine if the family is low-risk, but some examples of low risk situations are young families, teenage parents, parents or children with disabilities, and families in poverty. Referrals for In Home Based Services and Family Team Meetings may come from the Cabinet for Health and Family Services, Community Partners, families themselves, or you!
